1.  “worth dinner for four” means that the referring person will receive a gift certificate of $100 to the establishment corresponding with the magnet’s sticker upon qualification as defined below.  In the event that the prize is something other than dinner for four, such as a gift certificate, it will be marked as such on the sticker on the reverse side of the magnet. 

2.  “by referred patient” pertains to a person who is identified with the magnet at the time it is submitted,  and undergoes physical therapy in our office for a medically significant condition for which physical therapy is appropriate, for a minimum of six office visit treatments.  Reward is issued to the referring person upon completion of sixth physical therapy treatment visit by the referred patient.

3.  self-referral is allowed, using the magnet for oneself.

4..  magnet is invalid without the identification sticker adhered to the magnet side, which must have the referring person’s name and number.

5.  this offer may not be combined with any other promotional offer for the same referral, and one cannot be the referred person more than once in any twelve-month period.

6.  if the magnet is submitted and the referred patient does not complete six office visits, the magnet will be returned to the referring person to try again.
